Fuel flap release

The fuel release tab in my 2005 VZ sv6 has no effect and the fuel flap won’t open. What could be the problem?

Cable might have broken.

Start at the lever beside the sill

Twice now I have found the outer cable comes adrift/drops down from the lever housing and it has to be reinserted then a packer placed under it

on the floor to keep it in the housing, they are after all plastic.
